No longer do you have to rely on employers to provide coverage or pay high prices when you lose coverage or retire. The Integrity Simple Care Plan makes it easy to maintain a happy, healthy mouth with discounts on prescribed procedures, X-rays included, included annual exams and cleanings, and much more. This is not dental insurance, but rather, a solution to the confusing nature of insurance plans. There are no annual maximums or preexisting conditions clause. It’s simple. It’s affordable. It’s just what you need.

Membership Terms
Patients of record with the office of Integrity Dentistry may pay the annual membership fee at the first treatment appointment. The fee may be set up for monthly automatic withdrawal if requested*. The plan expires one year from the first membership payment. The membership fee is non-refundable. The Simple Care Plan annual fee is $249 for 1st family member ($209 for each additional family member of the same household).
An appointment is forfeited without a 24 hour cancellation notice.
The Simple Care Plan is not valid with any other offers, is non-transferable and cannot be applied retroactively.
The Simple Care Plan is a treatment bundle discount plan, not a dental insurance. It cannot be used with any other dental plans, dental insurance or financing programs (ie. Care Credit).
